Dental Insurance

The City offers dental insurance through Delta Dental of Wisconsin and pays 100% of the premium for full-time employees. Permanent part-time employees who work 20 or more hours per week are also eligible for this benefit at a prorated premium. Delta Dental covers 100% of the cost of a routine exam and cleaning every 6 months. Should you need dental services beyond your routine exams and cleanings (i.e. fillings, crowns, root canals, etc.), Delta Dental will pay for a percentage of the services up to a maximum benefit of $1,500 per person per year.  See the Delta Dental Handbook and Benefits Summary for more details.

Delta also offers an orthodontic benefit of $1,000 for each covered dependent child to the age of 19.

Providers

You can see the dental provider of your choice. However, if your provider is contracted with Delta Dental as a PPO provider or Premier provider, Delta will pay for a greater percentage of your services. The City offers vision insurance through EyeMed, underwritten by Delta Vision of Wisconsin and pays 100% of the premium for full-time employees. Permanent part-time employees who work 20 or more hours per week are also eligible for this benefit at a prorated premium.  Basic coverages in the plan include an exam every year, allowances toward lenses (eyeglasses or contacts) every year, and an allowance toward eyeglass frames every 2 years. Providers

You can choose EyeMed-contracted providers (examples include Target Optical, Shopko Optical, Lenscrafters, Wisconsin Vision, Eye Contact, Eyetopia, and more) or you can use non-contracted providers and submit a claim form with your receipts for a reimbursement up to the limits allowed.
