Case Management, Meal Sites and Home Chore Services
NewBridge services include:
· Case Management that connects case managers to seniors who need assistance accessing resources such as food, transportation, affordable housing, energy assistance and benefits.
· Nutrition Sites that provide socialization and meals to seniors throughout Madison and Monona.
· Home Chore Program that connects volunteers to seniors who need assistance with household chores so they can remain in their homes.
You still have the same case managers, same home chore coordinator and nutrition programs. The building at 4142 Monona Drive will still be accessible to you for the loan closet and meeting with your case manager
The ADRC (Aging and Disabilities Resource Center) of Dane County provides information about resources and support on all aspects of life related to aging or living with a disability and is a one-stop shop for older adults, people with disabilities and their families. ADRC staff are unbiased and knowledgeable professionals who listen to your concerns, help clarify your options and direct you to appropriate resources. The ADRC is also the access point for information about long-term care options and applying for public benefits. Services provided by the ADRC are free and available to all Dane County residents regardless of incomes or assets.
